If you have more than one receipt, please provide the most recent retailer and purchase date.

For each eligible item purchased, please enter the following:

  • the 12-digit numeric UPC barcode from the bottom or back of your product
  • the 12-digit alphanumeric redemption code from the sticker on the package
  • the price that you paid for this item (after coupons and discounts)

Each item entered will count as a separate rebate / sweepstakes entry (up to 5).
